Course Description: At least thirty percent of hospitalized patients are diagnosed with malnutrition when discharged from the hospital. Populations at greater risk of malnutrition include the elderly, low income, and minorities. The development of malnutrition is multifactorial and requires an interdisciplinary approach from providers throughout the care continuum to promote better outcomes. Early identification of nutrition risk includes the evaluation of factors associated with the development of malnutrition—including food insecurity, other social and environmental factors, disease progression, and aging.
Instructors will provide
- A simple bedside approach to identify and treat malnutrition in ambulatory practice settings
- Practical tips and resources to assist healthcare providers in identifying patients at risk of malnutrition, frailty, and/or food insecurity
- Real word examples that will help identify additional resources and opportunities for collaboration in the community.
Learning Objectives: Upon completion of this course, participants will be able to:
- Describe the current and anticipated burden of malnutrition, frailty, and food insecurity on individual patients and the healthcare system more broadly.
- Differentiate between the clinical presentation of malnutrition and/or frailty and expected age- or disease-related physiological changes when conducting nutrition risk screening.
- Identify at least 3 federal programs and/or community-based organizations with which you can collaborate to meet patients’ nutrition-related needs.
- List at least 2 bedside interventions available in your practice to address patients’ nutrition risk.
